Caseous necrosis, lymph node

    Observe the remaining lymph node tissue. Note scattered germinal centers with mantle zone, paracortical zone (Fig.1), lymph node capsule, and subcapsular sinus (Fig.2).

    Look for area of necrosis surrounded by granulomatous inflammation (Fig.3).

       Caseous necrosis is composed of necrotic cell and tissue with some visible outline of cells but not as preserved as being seen in cogulative necrosis (Fig.4), The name caseous is derived from the cheese-like appearance on gross examination.

      Around the caseous necrosis, there is granulomatous inflammation composed of epithelioid histiocytes , few Langhans type giant cell, lymphocytes and plasma cells. (Fig.5).
