Acute Metabolic Illness

Patient 3

An 8-month-old boy presented with fever, vomiting and lethargy.  He was born to a nonconsanguineous parents. He had an older brother who died at age 2 years because of severe acidosis. Another older sister had been normal.  PE: tachypneic, lethargic, and moderately dehydrated.

Laboratory data

CBC: normal Na 134, K 2.5, Cl 98, HCO3- 11, AG 25  BUN 2, Cr 0.6  LFT: unremarkable  Ammonia: 350 ?g/dl (25-94)  Urine FeCl3 & DNPH tests: -ve. Urine p-nitroaniline test: positive.
