From Songkla Hospital A one-month-old boy: lethargy for 3 d.  His parents were second cousins. His older brother died at age 3 months form severe metabolic acidosis without a definite diagnosis. However, the mother recognized the similar manifestation in her 2 children.  PE: BW 2700 g (his birth weight was 3200 g), moderate dehydration

Na 132, K 4.8, Cl 100, HCO3- 6, AG 26  BS 78, BUN 18, Cr 0.8  Ammonia 600 ?g/dl. Urine pH: 5.5, sp gr 1.026, ketone 2+
