A Thai male infant was born at term. His parents were second cousins. The patient’s older brother died of hypoglycemia and severe metabolic acidosis at age 5 months. PPHN & pneumonia: NICU 2+ weeks. At age 2 months, he developed lethargy. PE: dehydration, jaundice, and tachypnea.

CBC: Hb 9.52, Hct 28.3, WBC 1090, plt 19300 Urine pH: 6 and urine ketone: 2+ BS: normal, BUN 6, Cr 0.5 Na 133, K 4.4, Cl 94, HCO3- 17, AG 22 TB 6.46, DB 5.71, AP 220, SGOT 69, SGPT 95 Ammonia 492 ?g/dl (25-94) Urine FeCl3 & DNPH: -ve Urine p-nitroaniline: +ve
